Thinking about what matters most to you in your life is the starting point for making decisions about your health.  Important questions include-What do I do in my everyday life that makes my life worth living?  What have my experiences been with illness that are part of how I look at health care decisions?

Do you remember how you feel when you think back on important conversations you have had with people who matter to you?  You might say to yourself…that was a good talk.

Conversations about care you do and do not want especially when you are facing advancing, life-threatening illness are important “good talks” that need to go on throughout your life time.
Texas Talks is here advocating for you, people that matter to you and your healthcare provider to have these conversations.  We provide guidance about the entire process from starting the conversations to formally documenting your decisions.  The documentation ensures that health care providers can honor your wishes.

Patients may want to begin with our Planning Guide and our Communicating Your Choices Guide which can be found in our Resource Section of this site.  We also have a large selection of videos that can increase your understanding of meaningful conversations!

Providers may want to look at the of National POLST Form Guide for Professionals as well as the Frequently Asked Questions Guide found in the Resource Tab of this site.
